Please notify the surgery in writing of any change of your name, address of telephone number. You may download a Change of Address form here (this can be emailed to us at the surgery).
If you move outside our practice area, you will need to register with a surgery nearer your home. Please do not delay over this. The practice boundaries can be found on this website.
