You do not need a ‘fit note’ from a medical professional if you are off work, because of injury or illness, for seven days or less. In this instance, you may complete a ‘self-certification’ form. These forms are available from the reception team, or they may be available from your employer.

If you are absent from work for longer than this, you will need to contact the surgery to request a medical certificate or ‘fit note’. There is no charge for fit notes if you have been off work for more than seven days. The doctor may charge a fee if a fit note is requested earlier than seven days.

If you are self-isolating and cannot work because of coronavirus (COVID-19) you can get an ‘isolation note’ online from NHS 111 (https://111.nhs.uk/isolation-note/). You will not need to request a ‘fit note’ from the surgery.
