Chronic Kidney Disease (CKD) Quality Improvement Project 2025–26

Our practice is taking part in a national NHS Wales project to improve the care of people with chronic kidney disease (CKD).

What is CKD?

CKD means the kidneys are not working as well as they should. It is common, especially in people with diabetes, high blood pressure, or heart disease. Early detection and the right treatment can slow down kidney damage and reduce risks to heart health.

What are we doing?

Over the next few years, we will be:

  • Working through our practice lists and patient population to make sure people with CKD are correctly identified in our records.
  • Offering blood and urine tests (kidney function and protein checks) where these are due.
  • Reviewing medicines to ensure the best protection for kidneys and the heart (such as ACE inhibitors, SGLT2 inhibitors, and statins).
  • Providing advice and support on lifestyle changes, such as stopping smoking, healthy eating, and exercise.
  • Sharing information with patients on how to manage kidney health safely (including “sick day rules” for medicines).

Important to know

This project is a long-term piece of work and will take time to complete. You may not see immediate changes, but over the coming years we aim to steadily improve care and outcomes for all our patients at risk of kidney problems.

If you have diabetes, high blood pressure, or kidney disease, you may be invited for checks or a review as we work through this process.

Folate Deficiency

Vitamin B12 or B9 (commonly called folate) deficiency anaemia occurs when a lack of vitamin B12 or folate causes the body to produce abnormally large red blood cells that cannot function properly.

Red blood cells carry oxygen around the body using a substance called haemoglobin.

Anaemia is the general term for having either fewer red blood cells than normal or having an abnormally low amount of haemoglobin in each red blood cell.

Iron Deficiency

Iron deficiency anaemia is caused by lack of iron, often because of blood loss or pregnancy. It’s treated with iron tablets and by eating iron-rich foods. Iron deficiency, often leading to iron deficiency anemia.

It is a condition where the body lacks enough iron to produce sufficient healthy red blood cells. These red blood cells carry oxygen throughout the body, and a deficiency can cause fatigue, shortness of breath and other symptoms.

It’s a common condition, particularly among women, children and those with certain medical conditions.

Understanding Your Lipid Results and QRISK Score

Why Have I Had a Lipid Test?

A lipid test measures the fats in your blood, including cholesterol and triglycerides. These levels help assess your risk of developing heart disease or stroke. Your results are considered along with other risk factors to calculate your QRISK score.

What is QRISK?

QRISK is a tool recommended by NICE and used across Wales and the UK to estimate your risk of having a heart attack or stroke in the next 10 years. It uses factors like age, sex, ethnicity, blood pressure, cholesterol, smoking, diabetes, and other health conditions to calculate this risk.
